Job Description
We are looking for a compassionate and dedicated Care Home Nurse to join our team and provide high-quality nursing care to residents within a care home setting. The successful candidate will work closely with residents, families, care staff, and other healthcare professionals to ensure residents receive safe, person-centred, and respectful care.
Key Responsibilities

Provide high-quality nursing care to residents in accordance with individual care plans and clinical needs.

Assess, monitor, and record residents' health and wellbeing.

Administer and manage medications safely in accordance with prescribed instructions and organisational policies.

Support residents with long-term conditions, mobility needs, and other health requirements.

Recognise and respond appropriately to changes in residents' health or wellbeing.

Maintain accurate and up-to-date clinical and care records.

Work collaboratively with GPs, community nurses, pharmacists, therapists, and other healthcare professionals.

Support the development, implementation, and review of individual care plans and risk assessments.

Promote residents' dignity, independence, privacy, and choice at all times.

Provide guidance and support to care staff on appropriate clinical and care practices.

Follow safeguarding, infection prevention and control, health and safety, and medication policies.

Respond appropriately to emergencies and escalate concerns when required.

Communicate effectively with residents, relatives, and the wider care team.

Maintain professional standards and comply with relevant regulatory and organisational requirements.

Participate in training, supervision, audits, and continuous professional development.
